Using Ghostscript 7.0, GVPM 4.0, on OS/2, with letter-sized paper, on an
HP Laserjet 6L, I find:

 Barcodes begin 7mm from both right and left edge of paper.
 Barcode width is 11.5mm
 Top Ballot ID is 11mm from top, and 11mm from left/right
 Bottom Ballot ID is 8mm from bottom, and 11mm from left/right

That said, I found in preparing proofs of my book that my printer will
randomly shift placement by 1-2mm, especially up/down. And .5mm is
probably pushing the accuracy of my eyeball and ruler in making
measurements.
