It is very easy to modify the placement of the text and bar code
on the printed ballot. But my guess is that something happened going
from postscript to PDF to your printer. Did you use Acrobat Reader
to print out the ballot?

Can someone that can print postscript on letter sized paper try
printing this document and see how far from the edges the ballot
number and bar codes is printed?

There is a device dependent limit for how close to the edge a printer
can print. When I print the ballot on A4 (HP LJ5M), the bar code is
about 3.5 mm from the edge (both left and right). Of course, this
was when I had configured the printout for A4.
